Connect members to community resources, support care teams with outreach and education, perform non-clinical and social needs assessments, document resources, conduct telephonic and in-person outreach including home visits, and assist clinical staff as needed.

Manage and oversee the quality improvement program, analyze reports and compliance, coordinate with staff and departments to resolve complaints, improve procedures, prepare reports, monitor costs, and support budgeting and regulatory adherence.

Lead cross-product strategic initiatives, drive long-term product development and enhancements, integrate adaptive technologies and expert services, support sales/client customization, and ensure product financial performance and documented value propositions.
