Provide intake and assistance to older adults and adults with disabilities by assessing needs, referring to internal/external services, assisting with benefits enrollment (Medicare, prescription drug plans, Benefit Access), coordinating transportation, documenting confidential records, offering options counseling, collaborating with partners, and representing the agency at community events.

Respond to client inquiries, assess needs, provide referrals and benefit/Medicare enrollment assistance, coordinate transportation, perform options counseling, document confidential records, follow up on service connections, collaborate with partners, conduct outreach and presentations, and maintain required certifications.

Provide compassionate money management casework: conduct client intake and annual redeterminations, coordinate volunteers, maintain records for compliance, perform outreach and community engagement, and support clients and volunteers while ensuring program policies and documentation are met.
