Perform admission and MDS assessments for new and existing residents per the RAI manual; complete submissions and validation reviews; develop and monitor comprehensive care plans and nurse aide Kardex; attend IDT/Medicare/Medicaid meetings; review QMs and change-of-status to initiate OBRA assessments; complete required scales (Braden, Fall Risk), progress notes, and required documentation; coordinate with DON and consultants to ensure regulatory compliance and resident outcomes.

Lead and oversee nursing care delivery on skilled nursing units. Provide clinical leadership, education, infection control, wound and fall management, admissions support, chart audits, staffing oversight, complaint resolution, on-call coverage, and collaborate with DON/ADON to ensure resident needs and regulatory compliance.

Provide direct care to patients, assist in daily living needs, document care, and support the nursing team in a skilled nursing facility.
