Directs and evaluates home health service delivery, supervises clinical staff, conducts in-home visits, manages caseloads and productivity, reviews OASIS and care plans, supports reimbursement and quality outcomes, coordinates patient communications, leads case reviews and team meetings, assists with hiring and staff development, participates in on-call coverage, and represents the agency in community activities.

Coordinates daily office operations for a home health agency, supporting clinical staff through patient and clinical record maintenance, data entry, billing inputs, telephone and mail management, supply and medical equipment ordering, audit support, scheduling date tracking, onboarding assistance, and HR records management. The role also maintains confidentiality, supports community activities, and ensures office organization and regulatory compliance.

Provides one-to-one skilled nursing care in patients’ homes, follows physician-directed plans of care, monitors and communicates patient status, collaborates with care teams and families, supports safety and discharge planning, and completes timely clinical documentation.
