Teams at Care Partners

Provides comprehensive primary care to PACE participants, including evaluations, diagnoses, treatment plans, prescriptions, follow-up, documentation, and 24/7 assigned coverage. Collaborates on interdisciplinary care planning, quality improvement, EMR enhancements, and new medical programs. Supervises staff through hiring, training, performance evaluations, disciplinary actions, and team development. Schedule is Monday through Friday with no nights, weekends, or holidays except minimal phone-based on-call.

Provide comprehensive primary care to PACE participants (≈80% clinical): evaluate, diagnose, treat, prescribe, document, support EMR improvements, and offer minimal phone-based on-call coverage. Spend ~20% supervising and leading staff—hiring, training, performance management—participate in interdisciplinary care planning, quality improvement, and new medical program initiatives.

Provide outpatient psychiatric evaluations, medication management (in-person and telehealth), develop treatment plans with interdisciplinary teams, supervise NPs/PAs/therapists, participate in interventional psychiatry and clinical research, offer crisis support, and ensure documentation and regulatory compliance.
