Provide and oversee bereavement services at a regional branch, including counseling, support groups, outreach, education, and crisis intervention. Work as part of an interdisciplinary team, split between clinical and administrative duties, and assist the Manager of Bereavement Services in program planning and delivery. Position includes evening and occasional weekend work and requires local travel to patient homes and facilities.

Coordinate and manage hospice and palliative patient care as part of an interdisciplinary team. Assess patient and family needs, implement supportive nursing care per physician orders and organizational policies, provide symptom management across settings, and support families through end-of-life processes. Document care and collaborate with clinical leadership to maximize patient comfort and quality of life.
