Provide assisted living residents with personal care, mobility assistance, vital sign monitoring, emotional support, and communicate changes to nursing staff and families to promote resident dignity and independence.

Provide meal preparation, serving, and mealtime assistance for senior residents following dietary plans and safety standards. Maintain cleanliness, follow instructions, collaborate with care staff, and support resident nutritional needs and wellbeing.

Support residents by assessing needs, creating and documenting social service care plans, coordinating referrals and discharge planning, completing social history and MDS social service sections, writing progress notes, and communicating with residents, families, and the care team to enhance resident well-being.
