Provide community-based behavioral health case management for adults in supported housing. Responsibilities include individualized service planning, psychiatric rehabilitation, counseling, crisis intervention, medication support, daily living skills, housing assistance, resource linkage, documentation, compliance, transportation, and coordination with multidisciplinary teams and community providers.

Provides client-centered case management and navigation for individuals transitioning from hospitals, treatment programs, incarceration, or crisis services. Coordinates behavioral health, medical, housing, legal, and social-service referrals; supports housing applications, tenancy stability, and recovery goals. Maintains compliant EHR documentation for Medi-Cal, ECM, Community Supports, billing, audits, and quality assurance. Builds client and provider relationships, tracks closed-loop referrals, participates in care teams, and conducts local outreach across Sonoma and Marin Counties.

Provide community-based case management and psychiatric rehabilitation for individuals with mental health and substance use challenges. Develop service plans, support independent living skills, medication adherence, recovery, housing, budgeting, transportation, and community integration. Facilitate counseling and groups, coordinate referrals and crisis intervention, document services in an EHR, maintain compliance, and collaborate with multidisciplinary teams and external providers.
