Teams at Bristol Hospice

Provides after-hours hospice nursing support through evening, night, and weekend visits, admissions, telephone triage, patient and family coordination, staff dispatch, prescription refills, supply and DME ordering, documentation, and shift reporting. Requires compassionate care for terminally ill patients and grieving families, current RN licensure, hospice knowledge, flexibility, computer skills, physical stamina, and travel throughout the service area.

Supervises hospice clinical personnel and ensures appropriate delivery of patient care. Reviews referrals, diagnoses, prognoses, medications, records, and clinical services; coordinates hospice staffing; guides clinicians; supports quality improvement, employee hiring and orientation, education, policy development, and community activities. Requires collaboration with physicians and leadership while promoting compassionate, high-quality hospice care.

Provides direct hospice nursing care for terminally ill patients, including assessments, care-plan implementation, pain and symptom management, medication evaluation, documentation, education, and coordination with interdisciplinary care teams. Maintains aseptic technique, supports quality-improvement activities, and fulfills professional development responsibilities while delivering compassionate end-of-life care.
