Supervises hospice clinical personnel and ensures appropriate delivery of patient care. Reviews referrals, diagnoses, medications, records, and clinical courses; conducts home visits; assigns staff; guides clinicians; collaborates with physicians; supports hiring, onboarding, education, policy development, quality improvement, and community programs. Requires an active RN license, hospice home-care experience, supervisory or management experience, CPR certification, and a valid driver’s license.

Provides hospice nursing care by developing individualized care plans, assessing patient needs, administering medications and treatments, monitoring conditions, documenting care, educating families, and collaborating with interdisciplinary teams. Requires a valid state RN license, CPR certification, driver’s license, reliable transportation, and strong communication skills. The role includes patient and family visits with mileage reimbursement.

Provides hospice nursing care as an RN Case Manager, including individualized care planning, patient assessments, medication and treatment administration, symptom-management education, clinical documentation, condition monitoring, and collaboration with interdisciplinary teams. Supports patients and families with compassionate, dignified end-of-life care.
