Provide social work services for long-term/sub-acute residents: assess social/emotional needs, develop and revise care plans, document progress, participate in interdisciplinary care planning, and support discharge and placement planning.

Creates and updates master and daily staffing schedules for a care center, ensuring appropriate staffing levels for every shift and compliance with internal policies and external regulations. The role preferably requires Qualified Medication Aide certification in good standing and a clean state registry concerning abuse, neglect, or misappropriation of resident property.

Assist with meal service and delivery, maintain stock rotation and dining area appearance, ensure station and equipment cleanliness, and follow food safety and documentation standards while delivering excellent customer service.
