Provides RN home care coordination as part of an interdisciplinary team: attends IDT meetings, manages provider orders and schedules, conducts phone triage and home visits, documents care in the EMR, instructs participants/families, and supports quality assurance and nursing staff.

Provide safe medication administration and basic clinical care to assisted living and memory care residents. Duties include med setup and passes, narcotic counts, vitals, ADL assistance, documentation, scheduling doctor appointments, admitting residents, communicating with families and health staff, and maintaining resident safety and dignity.

Lead RM PACEs Quality Improvement and Compliance programs, ensuring adherence to CMS, HCPF, HIPAA and other regulations. Oversee policy development, audits, risk assessments, training, hotline investigations, reporting, QI metrics, survey readiness, and strategic planning. Collaborate with clinical and executive leadership, manage budgets, and foster internal and external partnerships to improve participant outcomes and regulatory performance.
