Coordinates care, benefits, treatment-plan support, and community services for adults with serious mental illness and substance-use disorders. Provides clinical assessments, counseling groups, home visits, welfare checks, transportation, documentation, billing, and follow-up after hospitalization. Maintains EHR records, tracks program outcomes, collaborates with healthcare and community providers, supports ACT clients, and assists with payee services. The role also helps train staff and promotes client engagement in behavioral health treatment.

Manages developmental disability service programs, including staff supervision, hiring, performance management, regulatory compliance, quality assurance, client documentation, community partnerships, direct services, training, and program finances. Ensures services meet Oregon administrative rules, contractual obligations, safety standards, and client-centered best practices while monitoring productivity, outcomes, revenues, and expenses.

Provides recovery coaching and case management to clients with substance use disorders. Responsibilities include outreach, relapse-support calls, transportation, home visits, sober activities, referrals, group support, documentation, drug testing assistance, and coordination with treatment teams. Requires lived experience with substance use disorder, at least two years of continuous recovery, strong harm-reduction skills, crisis judgment, confidentiality, and the ability to obtain required peer-support and safety certifications.
