The Resource Navigator conducts outreach, participant recruitment, intake, assessments, screening, goal planning, intensive case management, referrals, service coordination, basic-needs support, transportation assistance, and follow-up for families. The role documents services in Salesforce and PEIRS, supports workshops and caregiver groups, provides child-abuse-prevention education, collaborates with multidisciplinary teams, attends training, and maintains compliance with program and grant requirements.

Provides case management and transition services to youth ages 16–21, including intake assessments, individualized service plans, crisis management, referrals, documentation, and progress monitoring. Leads and mentors staff, ensures compliance with DFPS, grant, and organizational requirements, conducts case reviews, prepares reports, coordinates services, and presents on transition programs. Requires community engagement, public speaking, travel, and collaboration with agencies and multidisciplinary teams.

Coordinates community outreach, education, and recruitment of eligible families for program services. Builds relationships with families, healthcare providers, community leaders, and service agencies; provides referrals, facilitates meetings and training, coordinates events, maintains participant records in Salesforce, and ensures compliance with agency and regulatory requirements. The role also includes supervised transportation, frequent community outreach, documentation, and travel as needed.
