Provides outpatient endocrinology consultation, diagnosis, treatment, referral, follow-up, and patient education in collaboration with primary care teams. Maintains accurate medical records, participates in peer review and staff meetings, completes credentialing and continuing education, and supports provider training on common endocrine conditions. Requires a part-time schedule of 4–8 hours weekly, with occasional travel between health centers.

Conduct annual wellness and primary care visits for Medicare patients, including screenings, risk assessments, diagnosis, treatment, referrals, diagnostic testing, health education, documentation, and follow-up. Collaborate with multidisciplinary teams and supervising physicians where applicable. Maintain licensure, certifications, compliance training, accurate medical records, and participation in care-team meetings. The role is part-time at 16 hours weekly, with occasional travel to other health centers.

Coordinates specialty referrals, insurance authorizations, documentation transmission, and follow-up for patients. Leads and coaches Referral Coordinators, monitors work queues, trains staff, develops workflow materials, and supports Epic process improvements. Collaborates with providers, billing teams, care teams, and external specialty offices to resolve barriers and improve patient access. Ensures HIPAA compliance, handles patient concerns, and assists with clinic operational planning and process improvement.
