The intern also assists with administrative duties, follows CCM and RPM regulations, documents patient interactions, and participates in professional development and care conferences.

Provides ambulatory care pharmacy services for chronic disease patients, including medication management, counseling, adherence monitoring, remote patient monitoring, care-plan development, and therapy recommendations. Collaborates with physicians and other providers, supports patient enrollment and device management, educates staff and residents, and participates in research, literature reviews, data analysis, and clinical presentations. The role is clinic-based, requires proximity to the assigned location, and may involve limited regional travel.

Provides clinical pharmacy services for Chronic Care Management, Transition Care Management, and Remote Patient Monitoring. Builds patient panels, manages medications, interprets monitoring data, develops individualized care plans, recommends therapy changes, supports patient enrollment and device management, collaborates with providers, leads educational discussions, and precepts pharmacy residents. The role requires clinic-based work near Richmond with potential regional travel across affiliated practices.
