Provides direct nursing care to patients and residents in skilled nursing, sub-acute care, and long-term care settings. The role requires completing nursing assessments and care responsibilities while maintaining a caring, responsive approach. Applicants must hold an active LPN license in the state of practice and current CPR certification. Full-time and part-time evening and overnight shifts are available.

Lead and oversee the MDS/RAI process in long-term care settings. Ensure compliance with Medicare/Medicaid, OBRA, and payer requirements. Requires active RN licensure and collaboration with care teams to manage assessments, documentation, and quality outcomes.
