Teams at Assistance Plus

Provides in-home assistance with activities of daily living, including personal hygiene, toileting, mobility, transfers, nutrition, household tasks, transportation, and errands. Observes and documents clients’ physical and mental status, reports safety hazards and changes in condition, follows service plans and infection-control procedures, and maintains a safe environment. Requires travel between clients’ homes within a 30-mile radius and an irregular schedule including evenings and weekends.

Provides daily living support to adults with behavioral health diagnoses by monitoring independence, safety, and emotional well-being in home and community settings. Follows client care plans, transports clients locally, participates in clinical supervision, and completes required annual training. Requires a valid Maine driver’s license, insured vehicle, high school diploma or equivalent, basic computer and internet skills, and MHRT/1 certification, which the employer will provide training to obtain.
