Teams at Assistance Plus

Provides in-home personal care and daily living assistance, including bathing, grooming, toileting, mobility, transfers, nutrition, housekeeping, transportation, and errands. Observes and documents client condition changes, follows service plans and infection-control procedures, promotes household safety, and travels between multiple client homes within a 30-mile radius. The role requires an irregular schedule, including evenings and weekends.

Provides daily living support to adults with behavioral health diagnoses, helping them function safely and independently at home and in the community. Follows individualized care plans, monitors safety and emotional well-being, transports clients locally, participates in clinical supervision, and completes required training.
