Teams at Arbor Trail Rehab and Skilled Nursing Center

Coordinates pre-admission, admission, and discharge activities for residents and patients. Manages referrals, bed offers, payer pre-authorizations, assessments, admissions documentation, room assignments, tours, and family communication. Supports psychosocial care planning, care plan meetings, equipment approvals, level-of-care requirements, and resident concerns. Serves as backup to the Director of Admissions and maintains confidentiality and regulatory compliance.

Provide direct patient care in skilled nursing, sub-acute, and long-term care settings. Administer medications, change wound dressings, perform physical examinations and diagnostic tests, and collaborate with the care team to support patient needs.
