Provide solution-based case planning and intensive preventive case management to at-risk families referred by ACS. Conduct home and office visits, assess child and home safety, develop service plans, document case activity, coordinate community services, facilitate conferences, complete required trainings, and maintain compliance with ACS and AAFSC documentation and data systems.

Provide Solution-Based Casework case planning, management, and advocacy to at-risk families to prevent foster care placement. Conduct home and office visits, complete ACS-required documentation and FASPs, link families to community services, facilitate family conferences, maintain databases (PROMIS, Connections, Apricot), attend trainings, and support audits and supervision. Ensure compliance with ACS standards and Solution-Based Casework model.

Provide front-desk and office operations support, manage facility and supplies, coordinate room scheduling and safety procedures, and deliver resource navigation and referrals for clients seeking public benefits and community services. Assist with events, recordkeeping, interpretation, outreach, and administrative tasks to support program delivery.
