Provides case management for clients with substance use challenges, including assessments, individualized service plans, care coordination, advocacy, documentation, and progress reviews. Collaborates with therapists, families, and multidisciplinary teams to support recovery, independence, and daily living skills. May facilitate groups when SUD counselors are unavailable and must follow CARF and CMS documentation and billing practices.

Provides behavioral health case management for clients with mental health and substance use challenges. Responsibilities include assessing needs, coordinating care, developing and reviewing service plans, advocating for clients, collaborating with therapists and families, supporting independent living skills, and maintaining documentation and billing practices. The role requires a high school diploma, NPI number or Ohio Medicaid ID, background checks, and strong communication skills. A human services degree, CPR certification, and behavioral health experience are preferred.

Provides case management services for individuals experiencing mental health and substance use disorders in an outpatient behavioral health setting. The role supports clients while contributing to a recovery-friendly, community-oriented workplace.
