Provides skilled nursing care to patients in their homes, including assessments, wound care, medication management, chronic disease support, education, and family guidance. Works independently while collaborating with an interdisciplinary team, documenting care accurately, and supporting patient comfort, independence, and outcomes. Requires local travel throughout surrounding Maine communities.

Provide skilled, compassionate hospice nursing in patients' homes: assess physical, emotional, psychosocial, and spiritual needs; develop and manage individualized care plans; control pain and symptoms; educate and support families; collaborate with interdisciplinary hospice team.

Provide skilled nursing care in patients' homes following RN-developed plans: medication administration, wound care, assessments, documentation, patient/caregiver education, safety evaluations, interdisciplinary collaboration, and community health initiatives.
