Provide daily social, educational, recreational, and employment-readiness activities for people with developmental disabilities and mental health challenges. Support community-based learning, outings, employment preparation, social skills, behavioral de-escalation, personal care, transportation, and accurate daily documentation while coordinating with program staff and individual service plans.

Facilitates employment for individuals with intellectual, developmental, and mental health disabilities. Responsibilities include career assessments, job-readiness training, vocational profiling, employer outreach, workplace evaluations, accommodation education, on-site job support, advocacy, case documentation, and collaboration with support teams. The role requires travel among community worksites and partner agencies, flexible availability, physical assistance with transfers, and compliance with driving and background-check requirements.
