Facilitates employment for individuals with intellectual, developmental, and mental health disabilities. Responsibilities include career assessments, job-readiness training, vocational profiling, employer outreach, workplace evaluations, accommodation education, on-site job support, advocacy, case documentation, and collaboration with support teams. The role requires travel among community worksites and partner agencies, flexible availability, physical assistance with transfers, and compliance with driving and background-check requirements.

Provides daily social, educational, recreational, and employment-readiness activities for people with intellectual and developmental disabilities and mental health challenges. Supports community-based learning, outings, social integration, behavioral de-escalation, personal care, transportation, and employment preparation. Documents attendance, behavior, activities, and progress while collaborating with staff and following individual service plans.

Provide daily social, educational, and recreational services to individuals with developmental disabilities and mental health challenges. Plan and lead community-based outings, employment-readiness activities, and skills training; manage behaviors, assist with personal care, document progress, and transport participants safely.
