Teams at Aging Well Health Care, LLC

Provides skilled nursing care to home-health patients under physician direction, including assessments, treatments, care planning, education, documentation, progress reporting, and coordination with interdisciplinary staff. Supervises and evaluates home health aides, supports families, participates in case conferences and quality programs, and provides clinical education. Requires travel to patient homes and compliance with nursing, safety, and infection-control standards.

Administer employee benefits, payroll, enrollments, eligibility, deductions, reconciliations, insurance invoices, 401(k) programs, workers’ compensation reporting, audits, and compliance activities. Maintain records in Paylocity and QuickBooks, resolve payroll and benefits discrepancies, prepare reports, and support employees with benefit questions and education while coordinating with vendors, carriers, Finance, Payroll, and Human Resources.
