Manage planning, coordination, and execution of Cost of Care initiatives across the organization. Develop project plans, track milestones, facilitate cross-functional implementation, maintain governance artifacts, monitor performance metrics, prepare stakeholder communications, and support adoption of care models. Partner with clinical, operational, market, product, analytics, and technology teams to resolve issues and ensure successful initiative delivery.

Provide case management for high-risk patients across inpatient, ambulatory, and post-acute settings. Perform assessments, develop and implement Plans of Care and self-management plans, coordinate with PCPs and interdisciplinary teams, facilitate referrals and community resource use, perform home visits as needed, document interventions and outcomes, and ensure regulatory and quality standard compliance (HEDIS, NCQA, URAC).

Provide legal support for end-to-end payor and vendor contracting, focusing on Medicare Advantage and value-based care agreements. Draft and maintain templates, negotiate terms, manage post-execution issues, advise on regulatory impacts, coordinate cross-functionally (Finance, IT, Compliance), and support related provider, ACO, and clinical program contracting and legal research.
