Provides tenant-centered case management in supportive housing, including intake, assessments, support planning, care coordination, life-skills development, advocacy, benefits navigation, crisis and eviction-prevention interventions, community referrals, outreach, documentation, group activities, and outcome reporting. Collaborates with families, providers, agencies, and property management while maintaining confidential, accurate tenant records and supporting independent living.

Oversee supportive housing program operations, service delivery, regulatory compliance, staff supervision, hiring, training, performance management, quality assurance, reporting, incident response, and contract outcomes. The role coordinates services addressing tenants’ mental health, substance use, housing retention, employment, medical, and social needs while collaborating with property management, regulators, funders, and internal departments.

Directs social services for a shelter, supervising case management and related teams, coordinating assessments and independent living plans, ensuring regulatory compliance, and overseeing quality care. Develops programs addressing housing, substance abuse, mental health, and employment needs; tracks performance outcomes, manages documentation and incident reporting, liaises with providers and community organizations, conducts quality assurance reviews, and provides staff supervision, training, and crisis intervention oversight.
