Teams at Affinity Hospice

Provide psychosocial assessment, counseling, casework, pre-bereavement and bereavement care for hospice patients and families; develop and implement social work goals in the Plan of Care; attend interdisciplinary meetings, participate in on-call rotation, and assist with planning funeral, financial, legal, and healthcare decisions.

Provide administrative support to the hospice IDG clinical team by managing clinical records, ensuring regulatory compliance (state, federal, ACHC), maintaining record systems, supervising the records team, coordinating scheduling, performing audits, following up on unsigned physician orders, and supporting QA and other clinical support duties.
