Coordinate communication among patients, families, referral sources, physicians, caregivers, and clinicians to support smooth home care delivery. Manage scheduling and administrative processes, resolve questions, apply clinical knowledge to patient needs, and contribute to accurate, compassionate, patient-centered care. This is an in-office Monday–Friday role for a licensed Minnesota COTA or PTA seeking to transition from hands-on clinical work.
