Provide hospice nursing care including patient assessment, admission processing, care planning, implementation, reassessment, coordination with interdisciplinary team and physicians, clinical interventions, death pronouncement, family support, and timely documentation.

Provides private-duty home health care under a registered nurse’s plan, including direct patient care, documentation, education, care coordination, equipment preparation, and health instruction. The role requires compliance with the state Nurse Practice Act, aseptic practices, collaboration with interdisciplinary providers, and participation in professional development.

Manages hospice clinical records, ensuring accurate maintenance, confidentiality, regulatory compliance, and controlled release of patient information. Establishes records policies and procedures, supervises clerical personnel, completes post-discharge administrative audits, and supports clinical staff with documentation requirements. The role requires strong organization, communication, autonomy, and knowledge of information systems and healthcare documentation regulations.
