Provide hospice case management including patient assessment, eligibility determination, admission processing, care planning, implementation, regular visits, documentation, coordination with physicians and interdisciplinary team, supervise home health aide care, perform clinical pronouncement of death, and support families through end-of-life care.

Provide supervisory home health nursing visits, start-of-care assessments, in-service education, and coordinate with case managers to ensure quality patient care and independence at home.

Provide administrative support to clinical teams in home health: handle clinician/patient calls, pull and redact 485 forms, assist scheduling and incident reporting, and follow state/federal DOH guidelines.
