Provide targeted case management for individuals with serious and persistent mental illness, including hospital discharge planning, in-home and community visits, service linkage, monitoring, documentation, and collaboration with clinicians and community providers to support community integration and stability.

Coordinate and manage individualized care plans for clients, collaborate with providers and families, monitor progress, advocate for client needs, maintain accurate documentation, ensure regulatory compliance, and provide case management for programs supporting homelessness, substance use, or youth based on program assignment.

Provide face-to-face case management for individuals under Involuntary Outpatient Commitment, coordinate discharge and court documentation, collaborate with psychiatrists and community providers, conduct weekly client sessions, complete timely documentation, and participate in program meetings to support treatment adherence and stabilization.
