Teams at Abound Health

Build and maintain referral relationships with support coordination agencies, schools, and community organizations across Pennsylvania. Conduct outreach visits, presentations, and events; generate client referrals and warm leads; and coordinate handoffs to intake and care teams. Track referral activity, conversions, outcomes, and trends using CRM and reporting tools. Support referral partners, resolve concerns, promote Abound’s services, and ensure compliance with ethical referral practices and healthcare regulations.

Serves as the primary front-office contact for clients, families, caregivers, staff, referral sources, and visitors. Manages calls, emails, messages, inquiries, and routing of concerns; maintains a welcoming, organized, and confidential office environment; prepares training and meeting spaces; documents interactions; and identifies recurring issues or process gaps for leadership and continuous improvement.

Leads external relationship strategy and a regional partnerships team to grow referrals across multiple states. Builds relationships with healthcare, human services, payer, educational, and community partners; manages high-value accounts; represents the organization at events; develops outreach strategies; tracks performance in CRM and reporting tools; and collaborates with clinical, intake, marketing, and leadership teams to support client placements.
