Teams at Abby Care

Act as an embedded hospital liaison to build relationships with discharge planners, nurses, social workers and physicians, educate staff on Abby Care, drive high-volume patient referrals, streamline transitions from hospital to home, and rigorously track activity and pipeline in the CRM.

Lead and scale Abby Care's privacy and AI governance programs: own HIPAA and state privacy compliance, BAA program, AI use-case review and monitoring, incident response, vendor and outside counsel management, cross-functional legal partnership with Product/Engineering/Clinical, and hire and lead the privacy/AI team.

Provide comprehensive home-based case management for a caseload of patients: develop individualized care plans, supervise Home Health Aides, coordinate with PCPs and therapy providers, maintain timely electronic documentation, participate in on-call and holiday rotations, and work to improve caregiver satisfaction and patient outcomes.
