Provide clinical oversight and coordinate home-based care for a caseload of patients. Develop care plans, coach CNAs, document electronically, liaise with PCPs and therapy providers, participate in on-call and holiday rotations, and work to improve quality metrics like readmissions and caregiver satisfaction.

Own the payor strategy, build relationships with health plans, negotiate contracts, and develop payor infrastructure and analytics. Collaborate with clinical, finance, and product teams to enhance service offerings in home-based care.

Embed daily within assigned hospitals to build relationships with discharge planners, nurses, and physicians; educate staff on Abby Care, prospect eligible patients, drive referral volume, streamline hospital-to-home transitions, and rigorously track activities and pipeline in CRM.
