Provide clinical oversight and case management for a diverse home health caseload: develop individualized care plans, supervise CNAs/HHAs, coordinate with providers and families, maintain timely electronic documentation, manage escalations, participate in on-call rotations, and drive quality and readmission-reduction efforts.

Serve as the primary compliance owner for a 2-3 state cluster: localize policies, prepare for and respond to licensure surveys/audits, monitor state regulatory changes, deliver state-specific training, execute monitoring and remediation, and partner closely with operations leadership to ensure regulatory adherence across Medicaid home health, HCBS waivers, and family caregiver services.

Lead analytics across Operations, Clinical, Finance, Product, and Engineering; define metrics and reusable data models; build dashboards, forecasting and risk models; measure clinical quality and product adoption; improve analytics scalability, automation, and documentation; translate analyses into executive recommendations.
