Provides nursing and behavioral health services on an Assertive Community Treatment team. Responsibilities include client assessments, medication management and administration, service coordination, supportive therapy, symptom management, treatment-plan updates, physical-health referrals, and coordination of psychiatric and medical care for adults with serious mental illness.

Provides community-based behavioral health and substance use support to adults as part of a Community Support Team. Responsibilities include participating in team meetings and person-centered planning, encouraging recovery and wellness, modeling recovery values, assisting with crisis interventions, and helping individuals build skills for successful community living across emotional, social, housing, medical, educational, vocational, and legal domains.

Provide community-based peer support for adults with mental health and/or substance use disorders as part of a Community Support Team (CST). Participate in team meetings, person-centered planning, crisis intervention, and model recovery to encourage wellness, resilience, and community integration.
