Location: Village of Southampton, an Independent and Assisted Living with Memory Care senior community    

Pay Rate: $28.00/hr. - $34.00/hr.

Schedule:

Full Time: Must be available to work weekends and holidays

Part Time: Saturday and Sunday | 9:00a.m. - 5:00p.m.

Essential Duties and Responsibilities:
•    Administer medications safely and accurately per physician orders and regulatory standards
•    Oversee medication aides and conduct monthly medication reconciliation and audits
•    Coordinate daily staff assignments to ensure consistent resident care
•    Communicate effectively with physicians, families, and care teams regarding resident needs
•    Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support
•    Respond to medical and behavioral emergencies with sound clinical judgment
•    Complete clinical documentation including assessments, evaluations, and service plans
•    Document incidents and communicate promptly with leadership
•    Monitor care team documentation for accuracy and completeness
•    Support orientation and ongoing training of care staff
•    Assist with scheduling in collaboration with leadership
•    Attend required in-services and continuing education
•    Maintain professional appearance and hygiene- 
•    Promote respectful and effective communication across teams and families\
•    Perform other duties assigned to support resident care and operational excellence

Qualifications:
•    Current and valid LVN/LPN license in the state of employment
•    Basic Life Support (BLS) / CPR certification
•    Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care
•    Proficient in Electronic Health Records (EHR) and basic computer skills
•    Strong verbal and written communication skills
•    Demonstrated critical thinking and problem-solving abilities
•    Knowledge of HIPAA regulations and compliance standards
•    Ability to respond with patience and understanding to dementia-related behaviors
•    Physical ability to perform essential job functions with or without reasonable accommodation
