  • Manage performance of resident assessments, complete incident reports, and carry out direct care to new admissions, residents with acute illness, residents with a sudden change of condition, etc. Contact resident sponsor and/or home health/hospice/EMS about changes in resident condition in a timely manner as needed. Ensure the same through the supervision of direct care staff.
  • Ensure the Daily Routine for LPN Floor Supervisor, Daily Routine for CMAs, and Daily Routine for PCAs is completed/implemented by each LPN Floor Supervisor/CMA/PCA each working day.
  • Ensure operational effectiveness of medication rooms, med carts, and personal care stations, including ensuring appropriate medications, wound care supplies, and treatment supplies are present. Ensure medication rooms, med carts, and personal care stations are clean and orderly. Also ensure there is a proper quantity of needed medications in stock and as coordinated with the pharmacy, physicians, etc.
  • Become proficient in and ensure proper use of an internal system (Resident Manager) and communication log, while maintaining other such records as required to actively track incidents, illnesses, and happenings concerning the residents and staff. Make recommendations for improvement to management and monitor employee use for proficiency and accuracy.
  • Assist in establishing and implementing appropriate brand/image guidelines for FHB to effectively promote services and to meet/maintain targeted occupancy standards. Help ensure minimum census/occupancy levels for all departments are achieved as planned and oversee strategies and tactics used to ensure consistent and accurate actions are taken throughout the admissions/sales process.
  • Ensure that time and quality standards established by Fellowship Home are being met or exceeded and without error, including medication passes, general resident care, charting, filing, and other duties.
  • Ensure the completion of comprehensive resident care plans, involving participation from appropriate staff from all shifts, and ensure they are updated regularly to fit resident care needs, including activities of daily living (showering, walking, toileting, etc.).
  • Assist with staff hiring and orientation, training, continuing education, and staff development. Other personnel duties consist of supervising and coaching employees including helping to achieve goals, setting schedules, approving leave, reviewing performance regularly, counseling, and discipline.
  • Ensure daily attendance of scheduled staff that allows for adequate floor coverage within allotted budget parameters. Oversee the rotating on-call schedule for after-hours and holidays, and is available as needed for crisis management and emergencies. Be willing to fill any hourly position, as needed.
  • Maintain understanding and awareness of the Department of Community Health’s Healthcare Facility Regulation Division’s Rules and Regulations for Assisted Living Communities, interpret them appropriately, and apply them to effectively achieve and maintain operational effectiveness.
Qualifications

Degree as a Licensed Practical Nurse is required; a Registered Nurse is preferred. At least two years of experience in a supervisory role as an LPN/RN is required. Must have compassion and a desire to serve the elderly.


Physical Requirements:

  • Body Positions: Sitting, standing, squatting, kneeling
  • Body Movements: Lift (at least 35 lbs with or without assistance), bend torso, reach, turn head and torso, and/or push (at least 100 lbs with or without assistance) for up to 12-hour intervals.
  • Body Senses: Must have adequate use of eyes and ears; adequate power of speech.

Mental Requirements:

  • Reading: Must be able to read and comprehend written instructions pertaining to the care and oversight of the residents. Must be able to communicate verbally and in writing with peers and supervisors.
  • Language: Must be able to communicate in English, both verbally and in writing, well enough to give instructions, relay information, and carry on effective interchange with staff and residents.
  • Social: Must be able to relate well with others and provide a non-hostile workplace as well as receive direction from peers and supervisors with a positive attitude.

Skills Required

  • Licensed Practical Nurse (LPN) degree/license
  • Registered Nurse (RN) license
  • At least two years of supervisory experience as an LPN or RN
  • Proficiency with internal resident management system (Resident Manager) and accurate communication logging
  • Knowledge of Department of Community Health Healthcare Facility Regulation Division rules and ability to apply them
  • Ability to perform resident assessments, incident reporting, and timely communication with sponsors/home health/hospice/EMS
  • Ability to manage medication rooms, med carts, wound care and treatment supply levels
  • Physical ability to lift at least 35 lbs and push up to 100 lbs, engage in 12-hour shifts (sitting, standing, squatting, kneeling)
  • English communication skills sufficient for verbal and written instructions and documentation
  • Willingness to be on-call, work holidays, and fill hourly positions as needed for crisis management and adequate coverage
  • Compassion and desire to serve the elderly
