About the Role 

What is the VP of Integrated Community Partnerships?

The VP of Integrated Community Partnerships is responsible for leading the agency’s effort to enhance data sharing and integrate care coordination across different systems and sectors. This leadership position establishes and maintains trusted relationships with service providers and key stakeholders for the benefit of our shared clients. Leading the Community Information Exchange, the VP will oversee the division’s strategy, day-to-day operations, team supervision, quality of service, and effectiveness. This is a hands-on leadership role responsible for maintaining, utilizing, and growing the CIE partner network, and for driving integrated care internally and externally. The VP of Data Partnerships & Integrated Care ensures deeper integration with 211 operations initiatives through participation in our community care coordination technology system to share client information, manage referrals, and share client outcomes.   

What You’ll Do 

As a key member of the team, you will:

Community Engagement

  • Ensures the San Diego service provider community is engaged on the Community Information Exchange person-centered model by forming and sustaining partnerships and aligning initiatives that improve health, equity and quality of life. 
  • Meaningful engages with organizations from multiple sectors, including social services, health, education, employment, and government. 
  • Oversees collaborations with partner agencies and programs to consolidate and create more efficient service delivery between 2-1-1 and other programs within the community. 
  • Provides ongoing, timely, and effective communication with partners, listening to partners’ needs and challenges, and offering solutions through partnership with CIE.
  • Ensures continuity and accuracy of formalized data-sharing agreements, including Participation Agreements, Business Associate Agreements, and Data Use Agreements with key health and social service partners at all times.
  • Maintains and facilitates cross-sector data governance committees to maintain trust, privacy, and data security.
  • Increases the total number and diversity of active, participating agencies partnering on the CIE ecosystem.

Team Leadership

  • Implements and iterates strategic plans for partner engagement and expanded utilization of CIE.
  • Leads a strong and cohesive Community Information Exchange (CIE) Team.
  • Establishes, monitors, measures, and achieves clear outcomes and deliverables for the team and direct reports. 
  • Manages all aspects and expectations of the Community Information Exchange Team and daily operations; providing leadership and management to division staff to ensure individual and team success.
  • Oversees all aspects of people management within Community Information Exchange operations, including recruitment, selection, training, performance management, coaching, and motivation.
  • Builds effective working relationships with direct reports, peers, and leadership and works collaboratively across agency departments. 

System Enhancements & Impacts

  • Determines key metrics and evaluation processes to measure success, and implements methods to capture and use that data to continually improve outcomes. 
  • Works with Data and Informatics Teams to monitor and enhance CIE functionality and improve access to CIE data, develop partner utilization dashboards, track and report on data integrations and client outcomes, and expand potential for data to be used in research and publications. 
  • Works with Business and Partnership Development and Finance Teams to design services and provide data-driven proof of impact used to secure diverse funding and investments.
  • Works with 211 Service Delivery leadership to drive integrated care activities and maximize CIE utilization. 
  • Identifies and facilitates the implementation of system modifications to meet partner network needs.
  • Monitors systems in place to reduce the time it takes for a client to be referred and connected between CIE partner agencies.
  • Ensures Measurable improvement in tracking and addressing Social Determinants of Health (SDOH) across the community.
  • Promotes the CIE person-centered model locally, statewide, and federally via speaking opportunities, conferences, publications, etc. 
  • Disseminates unified impact reports reflecting combined health and social outcomes for clients and the CIE Network.

Requirements

  • Bachelor’s degree required; Master’s degree in Public Health, Social Work, Health Informatics, Public Policy, or Healthcare Administration preferred.
  • At least 8 years of progressive leadership experience in social services, healthcare, and public health sectors required.
  • General understanding of health privacy laws, including HIPAA, 42 CFR Part 2, and FERPA. Expertise in data sharing platforms, CIEs (Community Information Exchanges), HIEs (Health Information Exchanges), or care coordination software. 
  • Strong grasp of Social Determinants of Health (SDOH) frameworks and referral ecosystems.
  • Exceptional external communication skills with a history of influencing executive-level community stakeholders.
  • Experience in project management, setting and meeting goals and milestones required.
  • Demonstrated ability to translate complex data concepts into clear service delivery strategies.
  • Experience managing multi-agency coalitions or cross-sector steering committees.
  • Experience with Collective Impact models and Care Coordination models for client service is required. 
  • Experience with the health care and/or social services providers in the San Diego region is highly desirable.
  • Bilingual preferred.
