Schedule: Tuesday – Friday; 9:30am – 2:30pm

Location: SE Portland

Reports to: Housing Transitions Program Director

Compensation: $23.00 - $25.00 per hour


AGENCY OVERVIEW: 

Through the principles of Catholic Social Teaching, Catholic Charities of Oregon achieves lasting solutions to poverty and injustice by partnering with clients to meet their economic and educational goals, and to support their physical and social well-being. Catholic Charities is the official domestic relief agency of the Archdiocese of Portland in Oregon.

 

PROGRAM OVERVIEW:

Housing Transitions Program (HTP) is a relationship-centered program that utilizes the Housing First model. HTP assists in removing barriers to permanent housing through case management services, advocacy services, and Community Connection services to self-identified women experiencing homelessness who are 25 and better and who do not have minor children in their care. In addition to case management and outreach, HTP also operates a morning drop-in center where self-identified women can access immediate resources and community with their peers.


POSITION SUMMARY:

The primary focus of the Temporary Case Manager is to increase the program ability to provide services to the most vulnerable women who are living outside, and to help community members respond in authentic and helpful ways to their houseless neighbors.

 

PRINCIPLE DUTIES AND RESPONSIBLITES:

  • Participate in drop-in day space duties 3-4 days per week.
  • Ensure the integration of HTP services with other Catholic Charities programs.
  • Demonstrate responsibility for safety of clients, all assigned staff, and property; assist in crisis situations as appropriate; follow all established safety protocols and procedures; possess working knowledge of fire regulations and evacuation procedures.
  • Assume limited caseload of clients providing direct case management, retention, advocacy, and support services with the goal of finding or remaining in housing specifically clients who utilize HTP drop-in or are currently case managed by the agency.
  • Facilitate access to needed community services such as employment, physical health and mental health services, benefits assistance, and addiction treatment.
  • Respect and honor clients rights and responsibilities, hear client grievances, and demonstrate professional boundaries and ethics.
  • Maintain accurate client files and data collection, documentation in compliance with all applicable internal and externals regulations. Timely completion of program forms, paperwork, progress notes, quarterly summaries, financial documentation, and entering documentation in Client Information System (CIS) and Homeless Management Information System (HMIS).
  • Adhere to mandatory reporting laws and HIPPA requirements.
  • Engage people experiencing homelessness  at HTP day space, and connect them with services.
  • Provide information, referral, and advocacy to women outside.
  • Develop and maintain positive working relationship with community partners, funders, peers, local law enforcement, referral providers, and local neighborhoods
  • Integrate best practices and principles into program services and operations, including, but not limited to:
    • Housing First Model
    • Trauma Informed Care
    • Motivational Interviewing
    • Assertive Engagement
    • Catholic Social Teaching
  • Participate in trainings and meetings assigned by a member of management, their supervisor, and/or Human Resources
  • Adherence to Catholic Charities policies and procedures.
  • Perform other duties and responsibilities as assigned.

 

QUALIFICATIONS:

  • Bachelor’s degree in human services or equivalent lived and/or worked experience required.
  • 2 years’ experience providing case management, information, referrals, and advocacy
  • 1-year direct work with individuals experiencing homelessness preferred.
  • Trainings and Specialized Knowledge (If not currently certified, you must be willing to be certified within the first 60 days of employment)
    • Current CPR/First Aid certifications.
    • Narcan training
    • Blood Borne Pathogens
    • Mental Health First Aid certified.
  • Experienced in crisis intervention and de-escalation.
  • Knowledge of local resources; experience providing information and referrals.
  • Demonstrated competency working with people from diverse cultures. Ability to assess and treat clients in a culturally competent manner.
  • Ability to effectively work with standard office programs including Word, Excel, Outlook, Microsoft Teams and PowerPoint.
  • Demonstrate judgment and discretion in dealing with confidential matters.
  • Commitment to Catholic Charities mission to work in partnership with vulnerable populations to achieve lasting solutions to poverty and injustice.
  • Must be able to organize and prioritize work, be proactive, take initiative, follow through, and simultaneously manage multiple priorities to ensure goals are met in a timely manner.  High attention to detail required.
  • Excellent written and oral communication skills.
  • Strong analytical and strategic problem-solving skills.
  • Ability to work well in team setting, as well as independently; be flexible and adapt well to different dynamics in a fast-paced work environment.
  • Ability to lift 40 lbs.
  • Travel between sites in Portland required. Must have driver’s license, access to vehicle and ability to obtain automobile insurance at levels required by agency.

 

COMPENSATION AND BENEFITS:  

Compensation is commensurate with experience. Position is eligible for participation in 403(b) retirement plan and Catholic Charities of Oregon employees may qualify for the Public Service Loan Forgiveness government-sponsored education loan repayment program. Please note these benefits are subject to change.

 

BACKGROUND AND MVR SCREENING:

Satisfactory results from pre-employment criminal and civil background screening required. Motor vehicle records (MVR) review required as needed.
