Additional Job Description

High School Diploma required. Bachelor’s degree from an accredited college/university required in social work, psychology, counseling, or social welfare program preferred. 3-5 years of healthcare experience, discharge planning,

patient coordination and scheduling. Critical thinking skills and strong interpersonal skills essential. Excellent organizational and time management abilities, and able to work effectively within a multidisciplinary team. Skilled with

patient interaction, problem solving, navigation, and listening. This position has the opportunity to work primarily remotely in SC, with occasional travel to the MUSC Center for Telehealth in Charleston.
