Job Summary:
The Social Work Case Manager is responsible for assessing, coordinating, and monitoring the services and resources required by individuals or specific client populations. This position will also assist individuals, families, and communities in dealing with social, emotional, and financial challenges.

Duties/Responsibilities:
- Conduct comprehensive assessments to identify clients' needs, strengths, and challenges.
- Provide counseling, emotional support, and guidance to individuals and families facing difficulties, such as mental health issues, abuse, substance abuse, or financial problems.
- Advocate for clients' rights and access to resources within various systems, including healthcare, legal, and social services.
- Offer immediate support and intervention during emergencies or crises, ensuring the safety and well-being of clients.
- Engage in community outreach programs to raise awareness, provide education, and prevent social issues.
- Maintain accurate and detailed records of client interactions, assessments, and services provided.
- Develop individualized care plans based on assessment findings, setting achievable goals in collaboration with clients and their families.
- Coordinate and link clients to appropriate services, including medical care, housing, counseling, vocational training, and financial assistance.
- Regularly review and evaluate clients' progress towards set goals, adjusting plans to meet changing circumstances.
- Maintain accurate and detailed records of client interactions, services provided, and progress made.
- Provide support and intervention during crises or emergencies, ensuring clients' safety and well-being.
- Other duties as requested.

Education and Experience:
- Bachelor’s degree in social work or any human service-related field.
- LPN required; RN License preferred.
- Three years of case management experience is required.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift to 15 pounds at times.
