Social Services Coordinator | Skilled Nursing
Salary: $58,000-$64,000 annually (based on experience)
Schedule: Full-time days, Monday-Friday, with occasional rotating Sunday coverage
The Lutheran Home Skilled Nursing Community is seeking a compassionate and organized Social Services Coordinator to help residents and families navigate the healthcare journey. In this role, you will serve as an advocate, resource, and key member of the interdisciplinary care team, ensuring residents receive the emotional, social, and community support they need.
Key Responsibilities- Conduct resident interviews and psychosocial assessments
- Develop individualized support plans based on resident needs
- Assist residents and families with care transitions and discharge arrangements
- Coordinate referrals to behavioral health, counseling, and community resources
- Participate in care conferences and collaborate with clinical staff
- Maintain accurate documentation and resident records in PointClickCare
- Support resident rights, preferences, and quality-of-life initiatives
Required
- At least 2 years of social services experience in long-term care, skilled nursing, or senior living
- Previous discharge planning experience
- Working knowledge of PointClickCare or similar electronic health record systems
Preferred
- Bachelor's degree in Social Work, Human Services, Psychology, or related field
- Experience supporting Medicare and Medicaid populations

Schedule
Full-time, 9:00 AM–5:00 PM, Monday through Friday, with rotating Sunday coverage.
