What You’ll Do
- Coordinate outside psychiatric and psychosocial services to support resident care plans
- Conduct social service assessments and interviews to identify resident needs
- Provide counseling, advocacy, and discharge planning support
- Connect residents and families with community resources
- Encourage participation in individual and group programming
- Facilitate family involvement in care planning and transitions
- Assist residents with legal documentation and care coordination
- Maintain accurate documentation of social service plans and progress notes
- Collaborate with interdisciplinary teams to ensure comprehensive resident care
Required Qualifications
- Minimum of 2 years of experience in a Skilled Nursing Facility (SNF) in a Social Services or similar role
- Required experience: Discharge planning in a SNF
- Experience with customer service and resident/family support
- Related experience and/or training; or equivalent combination of education and experience
