Performance Responsibilities and Standards:

  1. Assign aides to cases requiring aide service, documenting in the patient schedule
  2. Notify patients, aides, contract agencies of changes in aide assignments or patient schedules
  3. Document changes in aide assignments or patient schedules
  4. Review reports such as, but not limited to: schedule reports, open case lists, aide availability, weekly hours, mileage spreadsheets, PTO and absence reports for assignments and/or reassignments
  5. Maintain accurate records and provide accurate information to aides
  6. Accept referrals for service and provide information to referral sources, contract agencies, patients and families, aides
  7. Coordinate weekend and holiday aide assignments and maintain accurate records of changes
  8. Record complaints received from patients and families, aides, contract agencies and assist in the investigation
  9. Report problems, complaints, updates and maintain close communication with the Clinical Operations Manager and on-call personnel
  10. Upon authorization from Manager, make arrangements for transportation for aides and provide instructions to aides
  11. Participate in patient / team conferences and quality improvement activities
  12. Promote a positive customer service approach to all individuals and actively foster open communication for a team approach to agency planning and problem solving
  13. Perform all other duties as assigned

Requirements:

  1. High School graduate or equivalent
  2. Excellent customer service and communication skills
  3. Positive attitude in working with people of diverse cultures and ethnicities
  4. Ability to work well with others in a fast-paced setting
  5. Accuracy with assignments and documentation
  6. Maturity, dependability and reliability
  7. Knowledge of the Central New York geographic region
  8. Ability to communicate effectively in English, verbal and written language skills
  9. Basic computer knowledge with proficiency in Microsoft Office or other Word processing, spreadsheet and database management
  10. Excellent organizational skills and ability to prioritize multiple tasks
  11. Compassion towards patients and families, their requests and needs as well as employee satisfaction
  12. Maintain patient protected health information
  13. Working knowledge of equipment such as: personal computer, multi-line telephone, copier and fax

Physical Requirements:

  1. Speech, visual and hearing ability sufficient to express and comprehend written and verbal communication
  2. Ability to sit or stand 90% of the day
  3. Frequent sitting, standing, walking
  4. Pushing, pulling, reaching, kneeling
  5. Bloodborne Pathogens Exposure Category III – Employee who rarely has contact with blood and body fluids.

Competitive Salary: [This position is an H05 non-exempt position with a min-max rate of: $18.65-$23.32/hour] with:
