We are a growing, fast-paced, physician-led office that is a leader in the field of kidney disease treatment looking to add a key member to our team, Remote Patient Monitoring (RPM)/ Chronic Care Management (CCM) Manager.
This role will be responsible for overseeing our RPM and CCM program. You will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices.
