At Nascentia Health, our Quality Assurance Coordinators play a vital role in ensuring exceptional patient care across our Certified Home Health Agency (CHHA) and Licensed Home Care Services Agency (LHCSA). This position is ideal for an experienced home care RN who enjoys reviewing clinical documentation, mentoring clinicians, identifying opportunities for improvement, and helping drive performance improvement and excellence in patient outcomes. What You'll Do:

  • Review clinical documentation to ensure compliance with state, federal, and agency standards.
  • Conduct quality assurance audits for CHHA and LHCSA clinical records, including OASIS documentation and coding reviews.
  • Analyze documentation trends and identify opportunities to improve quality, compliance, and patient outcomes.
  • Partner with clinical leaders and field staff to provide education, coaching, and guidance related to documentation, OASIS, coding, and best practices.
  • Follow up on documentation deficiencies and collaborate with clinicians to ensure timely resolution.
  • Support quality improvement initiatives and performance improvement projects across the organization.
  • Utilize quality reports and audit tools to monitor compliance and identify trends.
  • Assist with ongoing staff education and reinforce agency policies, regulatory requirements, and documentation standards.
  • Maintain current knowledge of Medicare, Medicaid, home health regulations, and quality improvement standards.
  • Collaborate with managers and interdisciplinary teams to promote a culture of continuous quality improvement.
  • Engages in meetings as an active participant, offering ideas and observations to support team goals and problem solving.
  • Perform additional quality-related responsibilities as assigned.

Job Qualifications:

  • Current New York State Registered Nurse (RN) license.
  • Minimum of two years of home health experience, or 4 years of quality assurance/improvement experience.
  • Experience with OASIS documentation and home health regulations, considered a plus.
  • Knowledge of Medicare, Medicaid, and home care documentation requirements.
  • Strong analytical and critical thinking skills.
  • Exceptional attention to detail and organizational abilities.
  • Excellent written and verbal communication skills.
  • Ability to work independently while collaborating effectively with clinical teams.
  • Commitment to delivering high-quality patient care and continuous improvement.
  • Ability to communicate effectively via telephone or digitally with clinical staff, colleagues, and other internal stakeholders.

Competitive Salary [This position is an S03 exempt position with a min-max rate of: $75,250-$87,138.92/annually] with:
