Lutheran Home in Arlington Heights is hiring an RN MDS Coordinator, $87,800–$99,700 a year. Monday through Friday, 8 a.m. to 4 p.m. No weekends, no on-call.
As RN MDS Coordinator, you'll manage the resident assessment process from start to finish, so residents get the right care and the facility gets accurate reimbursement for it. You'll work closely with nursing, therapy, social services, and dietary staff to keep assessments on time and on target.
What you'll do
- Lead the MDS assessment process for your assigned residents
- Review clinical documentation for accuracy before assessments are finalized
- Track assessment deadlines to stay compliant with federal and state rules
- Partner with the care team to build accurate, complete care plans
- Support Medicare, Medicaid, and OBRA assessment requirements
- Conduct chart reviews to catch documentation gaps before they affect reimbursement
- Help prepare for state surveys and support quality improvement work
What you'll need
Required:
- Active RN license in Illinois
- Graduate of an accredited nursing program
- At least 2 years of MDS 3.0 coordination and case management experience in a skilled nursing, rehab, or post-acute setting
Preferred:
- Prior experience in long-term care or skilled nursing
- RAC-CT certification, or willingness to earn it within 6 months of hire
