What You’ll Do: As a Clinical Manager, you will oversee the delivery of home healthcare services in the Farmington region, ensuring high-quality patient care and strong clinical leadership for our nursing and therapy teams. This leadership role includes staff supervision, caseload management, compliance with regulatory requirements, and coordination with physicians, hospitals, and community partners. Key responsibilities include:

  • Supervise and support clinical staff, including RNs, LPNs, therapists, and home health aides
  • Manage patient care services, including OASIS documentation, care plans, and physician communication
  • Oversee staff caseloads, scheduling, and performance evaluations
  • Ensure accurate, timely, and complete clinical documentation within the EHR system
  • Participate in patient case conferences, team meetings, and interdisciplinary care planning
  • Provide leadership for onboarding, staff education, and ongoing professional development
  • Conduct chart audits and monitor quality metrics to achieve positive clinical outcomes
  • Serve as liaison with referral sources, hospitals, and community agencies for care coordination and discharge planning
  • Participate in on-call clinical manager rotation and provide problem-solving support during off-hours as needed
  • Promote staff engagement, team collaboration, and patient satisfaction in alignment with Andwell’s mission

What You’ll Bring:

  • Graduate of an accredited nursing school; BSN preferred or actively enrolled in a BSN program
  • Current RN license in the State of Maine (required)
  • Minimum of 3–4 years of nursing experience in acute care with at least 1 year of leadership experience; home healthcare management experience strongly preferred
  • Current CPR certification (Healthcare Provider level)
  • OASIS knowledge and certification (or willingness to obtain within 2 years)
  • Strong leadership, communication, and organizational skills
  • Knowledge of reimbursement mechanisms, community resources, and interdisciplinary care models
  • Ability to analyze data, monitor quality outcomes, and support staff performance improvement
  • Valid Maine driver’s license, reliable vehicle, and auto insurance
