The RN Case Manager is responsible for providing nursing care to patients, including identifying and coordinating patient/family care with other members of the Interdisciplinary Team and health care providers.
RESPONSIBILITIES:
- Completes an initial assessment of patient and family to determine hospice care needs; provides a complete physical assessment and history of current and previous illness
- Uses health assessment data to determine nursing diagnosis
- Triages calls and assists patient/primary caregiver with problem resolution
- Teaches and counsels patients and families in meeting nursing and related needs
- Makes visits for symptom management and support as needed
- Modifies patient treatment plans as indicated by patients' responses and conditions
- Maintains current, accurate, detailed reports in accordance with policy
- Monitors, records and reports symptoms and changes in patients' condition to the IDG
- Provides health care instructions to the patient as appropriate per assessment and plan
- Obtains and receives physician orders as required
- Pronouncement of patients
Requirements
- Current license to practice as a registered professional nurse in Georgia
- Current and continued CPR certification
- At least one or two years of recent home care experience; hospice experience preferred
- Good verbal and written communication and problem solving skills
- Responsible, valid driver’s license; reliable, insured, automobile for making home visits required
