What You'll Do

As a Hospice RN Case Manager, you will:

  • Deliver skilled nursing care to patients in their homes/facilities in accordance with physician orders and the individualized plan of care.
  • Complete comprehensive assessments to identify each patient's physical, emotional, psychosocial, and spiritual needs.
  • Develop, implement, and update patient-centered care plans in collaboration with patients, families, caregivers, and the interdisciplinary team.
  • Educate patients and families about disease progression, symptom management, medications, and available resources to support informed decision-making.
  • Provide expert pain and symptom management to promote comfort and quality of life.
  • Coordinate care across disciplines to ensure seamless communication and continuity of services.
  • Maintain accurate, timely documentation in accordance with agency, state, and federal guidelines.
  • Advocate for patients and families while supporting their goals and wishes throughout the hospice journey.

What You'll Bring

  • Current Registered Nurse (RN) license in the State of Maine.
  • Minimum of one year of recent clinical nursing experience preferred
  • Current CPR certification.
  • Experience in hospice, home health, palliative care, or community health is preferred.
  • ANA certification in Home Health or Community Health Nursing is encouraged.
  • Excellent assessment, communication, and critical thinking skills.
  • Reliable transportation with valid driver's license and current automobile insurance.
