Seeking a Rehabilitation Specialist to join our interdisciplinary team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Oak Recovery Health Center. 

Full-Time Employment: 40 hours per week

Monday - Friday: 8:00am - 5:00pm

Location: 4451 E. Oak St., Phoenix AZ, 85008

Duties Include:

• Assist clients with identifying treatment and recovery goals, and provide supportive services to help clients reach their employment, education, and community involvement goals.
• Implement services identified in Individual Service Plans, and coordinate rehabilitation-related services for clients served on the clinical team.
• Communicate and document client progress toward their recovery.
• Embrace the recovery philosophy and promotes a safe recovery environment for clients.
• Use brief clinical interventions, Motivational Interviewing, and supportive listening skills to engage and support clients receiving services.
• Engage and provide direct support to clients’ natural support systems.
• Work in partnership with co-located Employment Specialists and RSA Vocational Rehabilitation Counselors, community program providers, and other parties involved to assist clients in maintaining employment, education, and other meaningful daily activities.

• A high school diploma/GED with a minimum of 3 years of behavioral health employment experience, or
  • An associate's degree in behavioral health with at least 3 years of behavioral health experience, or
  • A bachelor’s degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
  • A bachelor’s degree in the field of behavioral health and any behavioral health experience, or
  • A master’s degree or higher in the field of behavioral health.
• 1 year of supervised experience as a Rehabilitation or Employment Specialist, or Vocational Rehabilitation Counselor preferred.
• Computer Skills: Proficient in Microsoft Word, Excel, PowerPoint, & Outlook. Must be able to pass typing assessment with minimum 25wpm. 
• Knowledge in use of Electronic Health Records - NextGen is a plus.
• Bilingual in Spanish/ASL is desired.
• Excellent oral and written communication skills.
• Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.
• Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
• Must pass a TB Test.
