- Participates in the development of individualized rehabilitation plans.
- Ensure that all client appointments are set in a timely fashion
- Follows Caregivers Policy on transportation
- Participates in the collection of data for the comprehensive quality assurance plan to evaluate the program.
- Document all in-patient hospitalizations to clients participating in the Psych Rehab program
- Completes all documentation according to OMHSAS and Program guidelines.
- Maintain a caseload
- Maintain a flexible schedule. May have to work evenings or weekend for orientation to community programs.
- Plan, organize, and implement individual goal directed activities
- Develops a client driven curriculum consistent with client needs and choices
- Responsible for calling and returning phone calls to various team members of your client caseload.
- Responsible for scheduling and attending team meetings and STAR meetings as needed.
- Complete and submit billing authorization for clients
- Document after each session a note that describes how it helps each client work towards his or her goal.
- Schedule and meet with supervision every two weeks to update on caseload and any other issues.
- Other duties as assigned.
Competency and Experience [Knowledge, Skills and Abilities]
Required Qualifications/Experience
- High School Diploma, GED, or Higher Level of Education
- Two (2) years’ experience in human services, must include (1) year of experience in Mental Health direct service.
- Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
- Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
Preferred Qualifications/Experience
- Experience in Healthcare setting.
License, Certification & Clearances
- Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.
- Valid Driver’s License (if out of state hire, the record report from applicable state’s driver license department is required)
- Act 33 Clearance with renewal
- Act 34 Clearance with renewal
- Act 73 FBI Clearance with renewal
- Child mandated court Reporting Training within 30 days of hire or transfer
- Must obtain Certified Psychiatric Rehabilitation Practitioner (CPRP) within two years of hire or transfer
Position Type/Expected Hours of Work
- Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).
- Travel may be expected locally between System locations.
