PSYCHIATRIC REHABILITATION MOBILE

• Participates in the development of individualized rehabilitation plans.
• Ensure that all client appointments are set in a timely fashion
• Follows Caregivers Policy on transportation
• Participates in the collection of data for the comprehensive quality assurance plan to evaluate the program.
• Document all in-patient hospitalizations to clients participating in the Psych Rehab program
• Completes all documentation according to OMHSAS and Program guidelines.
• Maintain a caseload
• Maintain a flexible schedule. May have to work evenings or weekend for orientation to community programs.
• Plan, organize, and implement individual goal directed activities
• Develops a client driven curriculum consistent with client needs and choices
• Responsible for calling and returning phone calls to various team members of your client caseload.
• Responsible for scheduling and attending team meetings and STAR meetings as needed.
• Complete and submit billing authorization for clients
• Document after each session a note that describes how it helps each client work towards his or her goal.
• Schedule and meet with supervision every two weeks to update on caseload and any other issues.
• Other duties as assigned.

Competency and Experience [Knowledge, Skills and Abilities]

Required Qualifications/Experience

• High School Diploma, GED, or Higher Level of Education
• Two (2) years’ experience in human services, must include (1) year of experience in Mental Health direct service.

• Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice.
• Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.

Preferred Qualifications/Experience

• Experience in Healthcare setting.

License, Certification & Clearances

• Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.
• Valid Driver’s License (if out of state hire, the record report from applicable state’s driver license department is required)
• Act 33 Clearance with renewal
• Act 34 Clearance with renewal
• Act 73 FBI Clearance with renewal
• Child mandated court Reporting Training within 30 days of hire or transfer
• Must obtain Certified Psychiatric Rehabilitation Practitioner (CPRP) within two years of hire or transfer

Position Type/Expected Hours of Work

• Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).
• Travel may be expected locally between System locations.
