Title: Program Coordinator

Reports To: Home Delivered Meals & Transportation Program Manager
Summary: Responsible for the administration and coordination of Home Delivered Meals Program and DAAS Transportation; and support other programs of Nutrition and Senior Centers as required.
Essential Functions:
1. Coordinates the Home Delivered Meals Program and DAS Transportation Program. Provides quality service to clients by phone and e-mail according to policies and regulations.
2. Coordinates and/or performs assessments, surveys, evaluations and follow-up Home Delivered Meals and Paratransit participants.
3. Enters trip bookings, generates manifests and compiles reports in the Trapeze transportation system. Ensures on-time group van service is provided to DAS passengers.
4. Supervises and evaluates the Transportation Dispatcher and Group Van driver, and provides counseling and guidance as needed.
5. Implements and coordinates in-service training for group van drivers to meet the training requirements of San Francisco Paratransit.
6. Develops filing system and maintains service and client records. Performs data collection and prepares HDM and transportation monthly invoices and reports.
7. Attends meetings and trainings as requested by funding sources and events of other community organizations.
8. Handles vehicle administrative matters such as registration, insurance, placard application, etc.
9. Develops resources to support program operation, and makes recommendations regarding vehicle utilization and maintenance, home delivery meal supplies and equipment.
10. Assists in program activities as needed.
11. Performs other duties as assigned.
Qualifications:
1. Bachelor’s degree in Business Administration, Psychology or Human Services related field; and two years of supervisory experience.
2. Two years of experience working with the elderly and disabilities.
3. Must be bilingual in English and Chinese.
4. Proficient in MS Office and the Internet.
5. Must have and maintain a valid CA driver license and automobile insurance as specified in Self-Help’s policies.
